The Aging Care Specialist will be responsible for delivering dementia-focused curriculum for clients of the Specialized Aging Support (SAS), a program for individuals with intellectual and developmental differences who are living with or at high risk for developing Alzheimer’s disease-related dementia (ADRD).
- Under the direction of the Occupational Therapy Manager, deliver meaningful activities to adults with intellectual and developmental differences living with or at risk of Alzheimer’s disease-related dementia (ADRD).
- Facilitate dementia-specific curriculum for participants in the SAS program.
- Provide support and supervision to individuals with IDD living with or at high-risk of dementia in a respectful manner that results in the accomplishment of their personal goals; maintenance of health and safety, as well as fostering independent choices, and maintaining independence.
- Build relationships with clients, caregivers, and team of Arc Jacksonville staff working on the Specialized Aging Supports program.
- Engage in exciting and meaningful daily activities focused on fitness, nutrition, creative expression, life skills, and more
- Contribute to development and implementation of individualized care plans for each program participant
- Help create day center activities for participants, implement lesson plans, and contribute to monthly activities calendar
- Designated time to maintain daily documentation of participation and response to activities presented
- Receive specialized training in working with adults with intellectual and developmental disabilities living with or at risk of Alzheimer’s disease-related dementia
- Receive certification of screening tool to determine eligibility of program participants
- Participate in community outreach activities including recruitment of program participants and caregiver support groups
- Work with caregivers and families to provide support throughout progression of Alzheimer’s disease related dementia.
